Employee falls from tank top during cleaning operations and

At 4:15 p.m. on August 16, 2018, an employee was steam cleaning a Polar Tank Trailer, serial #31-26195. The employee fell from the top of the trailer tank and suffered injuries to the right side of his head and a fractured shoulder. The employee was discovered by a coworker unconscious and unresponsive. The employee died.
