Investigation Summary
At 12:00 p.m. on July 10, 2017, an employee was sitting in a pickup truck looking over work materials waiting for a top pick operator to arrive to his area of the terminal. The Top Pick operator drove around the opposite corner of a stack of Intermodal Shipping Containers. The far column of the stack of Intermodal Shipping Containers was only 2 high, where the closer column was three high. The Top Pick Operator nudged one of the 3 high containers when he was raising his boom, causing a cascade of shipping containers to fall on the pickup truck with the employee inside. The employee was crushed and killed by falling shipping containers.
