Investigation Summary
At 11:00 a.m. on October 6, 2020, Employee #1, employed by an electrical service s company, was engaged in interior work at a one-story commercial building. He was working from a 12-foot ladder and reaching up to the ceiling to repair a lig ht fixture. The employee sustained an electric shock to the fingers of both han ds. This caused him to fall from the ladder, a fall height of 12 feet, and stri ke his head against the concrete surface. Employee #1 sustained blunt force tra umatic injury to the head and was killed.
