Investigation Summary
At 1:28 p.m. on May 6, 2021, an employee was driving a tractor trailer loaded with demolished concrete to dump at a landfill. The employee lost control, possibly from loss of brakes, and jumped from the moving vehicle. The employee was killed when he impacted the ground.
