Investigation Summary
At 9:45 a.m. on July 14, 2017, an employee was going to mark the edge of a window sill of a ceiling roof window to be able to caulk and then replace the window which had been leaking. The employee stood on a closed step ladder that was leaning against a beam. The step ladder had been set on the platform of a mobile scaffold. The employee stood on the 4th step of the 6 ft. step ladder which was set on the platform of a mobile scaffold set at a height of 5 feet 7 inches above the concrete floor. The base of the window sill measured 16 ft. 5 inches above the concrete floor. The employee slipped and fell approximately 9 feet head first to the concrete floor. The employee was killed by fatal impact to the head after falling onto a concrete floor.
