Investigation Summary
At 10:30 a.m. on August 20, 2018, an employee was working on a truck's rear wheel and drive shaft while on a service lift and raised 7 feet. The employee attempted to stop the vehicle when it shifted and rolled completely off of the elevated ramp. The employee was crushed when the truck fell and struck his head and upper torso, killing him.
