Investigation Summary
At 1:00 a.m. on December 20, 2017, an employee was on the ground level when asked by the site superintendent to join him on the roof of the building to assist in installing steel decking. The employee climbed a ladder to access the roof. Once on the roof the employee walked over to where the superintendent was fastening decking to the roof purlins. The employee fell through an insulation covered opening to the concrete flooring below (a distance of sixteen feet). The employee sustained injuries to his head upon impact with the concrete floor and was killed.
