Investigation Summary
At 3:15 a.m. on May 31, 2017, an employee was operating a forklift, working alone during the night shift. He was in the 9th hour of the second day of a 12-hour shift rotation cycle. He dismounted a 1998 Toyota powered industrial truck (PIT), Model 426FGCU20 with Serial #79220 with standard fork attachments and then walked forward in front of the PIT to remove a piece of plastic that was caught on the "tipper" machine. As he walked toward the tipper the idling PIT he operated continued in a forward orientation and followed behind him toward the tipper. The mast of the PIT struck his back and pinned him against the leading edge of tipper machine. The tippers horizontal edge is approximately 3/8 thick is four feet above the floor level. It was approximately 9 1/2 minutes before he was discovered by another coworker. Resuscitation efforts were not successful.
