Investigation Summary
At 3:00 a.m. on July 26, 2018, an employee was installing a new LED light fixture from the ceiling. The employee used a scissor lift to access the area then exited the lift to perform the wiring portion of the task. As this task was being performed from an elevated location without the benefit of fall protection, the employee fell 23 feet below and struck a laundry conveyor member and ultimately the ground level below. It was later determined by the Lucas County Coroner that the deceased suffered a heart attack at some point while performing the wiring task in this condition.
