Investigation Summary
At 11:30 a.m. on October 30, 2017, an employee was off-loading a back hoe and storing the chains and equipment used to secure the track hoe onto the semi-tractor. The employee was in the process of storing the transport chains into a storage box on the passenger side of the semi-tractor when the driver began moving the tractor forward. The employee apparently was pulled under the drive wheels of the tractor and subsequently crushed under one of the drive wheels. It appears that the driver may not have known that the employee was by the drive wheels due to not checking behind him or some other oversight prior to moving the tractor. The employee was crushed under the semi-tractor drive wheel and is killed.
