At 8:00 a.m. on February 12, 2018, an employee was inspecting an 1,800 gallon stainless steel tank of a GVM Prowler, Model E-325, Agricultural Sprayer. As the employee walked between the tank he was inspecting and another tank, looking for hairline cracks, he became caught between the two tanks and was killed. The incident investigation revealed that during the employee's inspection, a coworker pressurized the other tank, causing the employee to be struck in the forehead by the stainless steel wall of the pressurized tank. The employee died as a result of the head injury.
