Investigation Summary
At aproximately 12:30 p.m. on January 31, 2020, an employee was inspecting a machine for a part number. Then, the machine started up unexpectedly and a hurtling 600 lb roll of paper and shaft struck the employee. The employee was struck in the chest and was killed.
