Investigation Summary
At 10:15 a.m. on February 8, 2021, an employee completed removal of a breakout wrench and began to remove a second breakout wrench when the drill stem the breakout wrench was attached to, began to spin. The wrench struck the employee twice, threw the employee underneath the drill stem, and the employee was killed.
