Investigation Summary
At 2:33 p.m. on July 31, 2019, an employee was removing an encoder to align the spindle. The employee crawled inside the machine to reach the encoder, the machine alarmed, the Y-axis started to drop, and the entire spindle assembly dropped down, crushing the employee.
