Investigation Summary
At approximately 11:18 p.m. on January 25, 2020, Employee #1 was working from an elevated work platform to secure a lid of a "sand box" trailer. The employee was walking from the safety rack system back onto the main catwalk after securing the lid. A coworker, who was the driver of the truck, began to drive away. The trailer of the truck hit the safety rack system, knocking Employee #1 down. Employee #1 was, then, run over by the truck resulting in head and arm trauma. He was killed.
