Investigation Summary
At approximately 1:25 am on June 6, 2017, Employee #1 was attempting to park the lift beneath the Eddy Current conveyor system. The height beneath the Eddy Current conveyor system from the ground to the I-beam was approximately eight (8) feet high. Employee #1 was a lead employee on the night shift. Two other employees were instructed to pick up tools and debris. Employee #1 started the lift and began traveling in reverse toward the conveyor system. The boom platform was lifted approximately 1.5 ft. to 2 ft. high from the ground during travel. With the conveyor system structure at his back, Employee #1 struck the I-beam of the conveyor and was pinned between the I-beam and the controls of the lift. The I-beam struck Employee #1 in the back of the neck forcing him forward against the control panel killing him instantly. Employee #1's injuries included a broken neck, broken ribs, and internal bleeding.
