Investigation Summary
At 3:00 p.m. on November 15, 2017, an employee was securing a waterproof tarp to a common wall from a ladder. The ladder was not set properly and the base slide away from the wall while simultaneously sliding down the wall. The employee fell onto a concrete slab and sustained severe head and chest trauma and was killed.
