Investigation Summary
At 12:00 a.m. on May 15, 2020, an employee used a twenty ton air bottle jack to raise a Blaw-Knox Road Widener to conduct repairs underneath the equipment. The employee moved underneath the widener without using blocks, cribbing or chocks to prevent movement. While under the widener, the jack kicked out and the widener fell onto the employee's head and was killed by crush injuries.
