Investigation Summary
At 10:34 a.m. on October 25, 2018, Employee #1 was operating a Nissan Uni - Carrier CTIF2 while unloading a semi-trailer. Employee #1 stood up and a stuck his head into the mast with the forks elevated. It appears that Employee #1 hit the controls causing the mast to lower, striking and entrapping his head. There was no load on the forks at the time of the incident. The employee was pronounced dead at the scene.
