Investigation Summary
At 1:00 p.m. on May 7, 2018, Employee #1 was installing a prefabricated wall panel. The wall panel began to fall out and away from the building. The panel fell, with Employee #1 holding on, to the ground. Employee #1 was killed upon impact.
