Investigation Summary
At 6:20 p.m. on October 12, 2020, an employee entered an automated rear seat loa ding cell to free a jammed pallet containing an automobile seat from the elevate d loader connected to a rack being loaded. Upon freeing the jammed pallet, the c ell resumed it's cycle striking and crushing the employee. The employee was kil led.
