Investigation Summary

At 11:20 a.m. on May 28, 2024, Employee #1, a CDL truck driver, was backing his 18-wheeler flatbed trailer into the loading area where reels were staged. The reels were stacked 4 stacks high in two rows next to the loading area where Employee #1 had parked. Wooden reels were constantly being loaded and unloaded via forklifts. Employee #1 exited his truck to attach the truck to the trailer. Employee #2, a forklift operator, came over to move the reels to a different trailer. As Employee #2 positioned the forks to lift the reels, it caused an adjacent stack of reels to fall. The forklift operator immediately jumped off the forklift to aid Employee #1. Emergency services were called, and Employee #1 was transported to the hospital, where he died from head injuries on May 30, 2024. Three additional employees were hospitalized for injuries, and three employees were treated without hospitalization.
