Investigation Summary
At 11:20 a.m. on June 19, 2018, Employee #1 was preparing to attach a chain to a trailer chassis for rotation to a new position. The employee removed the C clamps holding the chassis in place. It was not secured on that end of trailer. The trailer shifted and tipped over, striking the employee, and causing a deep laceration to his chest, killing him.
