Investigation Summary
At 10:45 p.m. on March 30, 2020, Employee #1, employed by a steel erection company, and Coworker #1, his supervisor, were engaged in steel erection at a multiemployer construction project, a new manufacturing structure. Employee #1 was in the basket of an aerial lift and installing/welding steel cross bars to an overhead joist. He became unresponsive, possibly from an electric shock. The supervisor, using ground controls, proceeded to lower the basket. When the basket was approximately 25 feet above the concrete floor, Employee #1 fell backward from the basket and landed head first. Emergency services transported Employee #1 to the hospital, where he was treated for a fractured skull. Employee #1 died during treatment.
