Investigation Summary
At approximately 9:30 a.m. on September 25, 2017, Employee #1 was working on a workover drilling rig. As he worked on the rig, the drilling rig topped over. The support structures of the rig struck Employee #1, causing crushing injuries to his body, killing him.
