Employee is crushed and killed by falling bin

At 11:30 a.m. on April 28, 2020, an employee was working for a firm that made cable reels and other wooden and plastic reels. He was operating a Clark forklift. He was carrying a small garbage bin elevated by the forks. He drove this forklift toward a large dumpster bin. He parked the forklift directly in front of the large dumpster bin. He left the engine running. He put the forklift in neutral and set the emergency brake. The concrete surface that the forklift was parked on was on a five-degree incline. The forklift's wheels were not chocked or blocked. The employee dismounted the forklift and walked into a small open space gap between the small garbage bin and the large dumpster bin. While he was in this small gap, the small garbage bin slipped off the forks. The small garbage bin crushed the employee in the back and pinned him against the wall of the large dumpster bin. The employee was killed. He was pronounced dead on the scene by the Los Angeles County Fire Department at 11:33 a.m. on Tuesday, April 28, 2020. DOSH learned of this event at 1:03 p.m. on Tuesday, April 28, 2020, after it was reported to the Cal/OSHA Long Beach District Office. At 2:15 p.m.on Tuesday, April 28, 2020, the Division held an opening conference with the employer.
