Employee is killed when struck by falling mast

At 3:00 p.m. on May 9, 2018, Employee #1 was installing a five foot section of a buck hoist mast in the fifteenth floor. The employee only placed two of the four needed bolts to assemble the mast section. The mast pivoted on the two bolts, and the cab of the buck hoist fell, along with a section of the mast, striking and killing Employee #1.
