Investigation Summary
At 12:00 p.m. on September 13, 2018, an employee was cleaning up the work site in preparation of departing the site for the day. The employee, operating a Vermeer S725 mini skid steer, had disconnected a bucket attachment from the loader and attached a grappler attachment in order to pick up five sheets of 4 foot by 8 foot plywood that was used to prevent the tracks of the loader from tearing up the resident's yard. The grappler's means of attachment to the skid steer was not properly secured. The grappler and plywood fell and one of the spikes on the grappler fatally pierced the employee's head.
