Employee Falls From Platform And Is Killed

At 12:19 p.m. on August 14, 2020, an employee was a part of a maintenance team w orking to tighten a nut on a granulator. The employee and the coworkers were usi ng a spanner wrench with cheater bar that was approximately 106 inches long to i ncrease leverage. As the employee and the coworkers were torqueing the nut with the spanner wrench, the lug on the spanner wrench sheared off. The employee was standing on a platform and fell backward. The employee struck his head on the fl oor and was killed.
