Investigation Summary
At 10:30 a.m. on July 13, 2019, Employee #1 and a coworker, employed by a construction company, were working at a residential job site. Employee #1 was on a ladder near the peak of the roof, and the coworker was on the ground and holding the ladder. The coworker stepped away from the ladder to point out an area on the roof that still needed work. One foot of the ladder came off the ground and the ladder fell. Employee #1 fell. He was transported to the hospital, where he died from his injuries.
