Investigation Summary
At 8:30 a.m. on June 8, 2018, an employee finished installing additional weights to the counterweights on elevator car D and was preparing to do the same to the next car, E. Out of sight, he was in verbal communication with coworker #1 who was located in a third elevator car, F, and out of contact with coworker #2. Coworker #2 was on the 8th floor waiting to meet with someone when he decided to come down to the ground floor. Coworker #2 entered elevator car E and descended without making any contact with his coworkers, when he heard someone cry out. Inspecting the situation, the employee was found crushed between a counterweight and a horizontal I beam.
