Investigation Summary
At 3:28 on September 28, 2018, an employee was installing screws in a metal building while standing on a 6 foot step ladder. The employee lost his balance and slid into the ladder, striking his right side. The employee was diagnosed with a strained and bruised chest wall muscle. The employee was a his residence when he lost consciousness and stopped breathing. The employee was transported to CAMC Teays Valley, where he was pronounced dead at 3:38 p.m.
