Investigation Summary
At 2:30 p.m. on July 31, 2018, Employee #1 was trying to locate an underground water tap that was beneath the northbound traffic lane of the M-25 interstate. The posted speed was 50 miles per hour. While bent over, looking at the reading on a locating instrument, the employee was struck by the left front corner of a passing motor vehicle. Employee #1 was hospitalized and later died from severe internal injuries.
