Investigation Summary
At 12:00 p.m. on November 22, 2021, an employee was on the top level of the shavings bin that was equipped with an auger. The machinery had an opening of approximately 20 to 24 inches, and it is believed that the employee was attempting to get the measurements needed to fabricate a cover for the opening. The machinery was operational, and while attempting to gather measurements, the employee was pulled into the auger. The employee was killed by the energized auger.
