Investigation Summary
At 6:00 p.m. on October 20, 2017, Employee #1, a warehouse worker, and his coworker, also a warehouse worker, were lifting boxes containing couches and love seats to the top level of a storage rack. The coworker operated a Nissan, LP forklift with a 42 inch wide by 48 inch long pallet attached to the forks, to elevate Employee #1 approximately 12 feet to the top level of the storage racks. According to the coworker, he and Employee #1 had lifted 14 sets of furniture. A set consists of two boxes, one couch box and one love seat box. The coworker stated that Employee #1 was sitting on a love seat box that was stored on the pallet attached to the forklift. There were also two couch boxes on the pallet. Employee #1 stepped off of the forks and onto the top level of the storage rack. He fell backwards onto a sofa box and then to the concrete floor. Employee #1 was transported to the hospital. He was pronounced dead two days later.
