Investigation Summary
At approximately 4:36 p.m. on December 17, 2020, an employee was tasked with performing their daily responsibilities which included, but were not limited to, checking inventory and loading products onto delivery trucks. The employee was walking northward down the center pass of the shipping area. At the same time, a coworker was traveling down the center pass while operating a Hyster 360 forklift (Model H360HD2: Serial No. J019E01778L) in a forward northward direction without a load. At approximately 2:15 PM, the traveling path of the forklift and the employee intersected. The rightmost fork of the forklift struck the employee in the back of the leg. The employee fell onto the fork of the forklift in a sitting position. As the forklift continued to move forward, the employee spun off of the forks onto the ground immediately in front of the front right tire of the forklift. The forklift continued to move forward and ran over the employee. The employee suffered a crushing injury to the head due to being struck by the forklift and died from these injuries.
