Investigation Summary
At 9:15 a.m. on July 6, 2018, Employee #1 was involved in a disciplinary meeting with supervisors. The employee was informed that he would be placed on administrative leave, and was instructed to unload and clear his Glock model 22 service weapon. Instead, the employee unholstered his weapon and pointed it at his head. After a brief discussion, the employee shot himself in the head.
