Investigation Summary
At 1:30 p.m. on March 12, 2018, an employee was airing up a tire that was being serviced. The multi piece rim was not in a restraining device when the air pressure was placed in the tire, resulting in a tire explosion. The employee and a coworker were struck by the tire rim.
