Investigation Summary
At 9:45 a.m. on June 8, 2018, an employee was sitting behind the rack of pipes, near a storm drain line. A coworker did not see the employee sitting behind the rack. The operator attempted to lift a load to move elsewhere. One of the pipes in that move fell, breaking the pallets that were set to hold it when stored. It fell between two other pipes, causing the stack to move out of place with two falling out of the storage rack onto the employee sitting behind the rack and killing the employee.
