Investigation Summary
At 8:30 a.m. on July 13, 2022, an employee was assisting the owner with repairs/maintenance of a skid loader. The lift arm fell onto the employee's head. The employee was pronounced dead at the scene by emergency personnel. The employee was struck by the lift arms and fork attachments of the skid loader.
