Investigation Summary
At 7:25 a.m. on August 28, 2017, an employee was walking on the top of a waste water treatment plant grit tank wall. The employee fell from the top of the wall and landed head first on the ground made up of loose gravel and stone, suffering fractures to his head and skull. The employee was hospitalized and later died from his injuries on August 29, 2017.
