Investigation Summary
At 11:00 a.m. on December 11, 2017, two employees were installing an insulation board on the exterior of the elementary school from an elevated platform of a mast climbing scaffold system. Both employees were thrown from the scaffold and fell to the ground when the mast that was supporting the insulation board fell over. Employee #1 suffered multiple injuries including skull fractures and was killed. Employee #2 was hospitalized with injuries to his back, right leg, and right knee.
