Investigation Summary
At 9:30 a.m. on November 12, 2019, Employee #1, a 25-year-old male, was working for an environmental consulting service. He had been working for the company for fifteen months. This was his second day working at this jobsite. He was working inside a man lift to install a gas analyzer in an exhaust stack. He was operating a JLG Model Number 860SJ telescopic boom lift. He was preparing to perform air emission testing on three stationary reciprocating combustion engines. They were located inside the stack yard and adjoining fenced area. The stack yard contained the office trailers, gas compressor skid, and horizontal radiators for the engine coolant. These items prevented the employee from positioning the lift inside the stack yard. The employee raised the lift over the 2.4-meter-high (8-foot-high) fence to extend the boom southward approximately 18.3 meters (60 feet) to reach the Number 3 exhaust stack. He did this in order to install a gas analyzer and position a moister train on the outside of the exhaust stack. After the employee installed the testing equipment, he rotated the lift backward from the stack. As he did this, he came into contact with the inside phase conductor of a 19.9/34.5kV three-phase overhead power line located on the 7.2kV line. He sustained burns, and he was electrocuted.
