Investigation Summary
At 2:30 p.m. on December 20, 2017, an employee was holding a tag line on a traffic signal arm that was being moved with a Altec digger/derrick. The rope on the winch broke and the employee was struck by the load. The employee was killed from blunt force trauma to the head and upper body.
