Investigation Summary
At 11:45 p.m. on January 18, 2018, an employee was operating abrasive blasting equipment to prepare industrial rollers for plating. The employee was working alone. For an unknown reason, the employee became entangled with the equipment at its coupler location and was killed. The employee was later found with his shirt and upper portion of his torso wrapped around the coupler and shaft (PTO and journal).
