Investigation Summary
At 8:05 a.m. on February 12, 2020, an employee was working from an extension ladder to clean the gutter of a single family residential home. At some point, the ladder kicked out from under the employee who fell backwards twenty feet and struck his head. The employee was killed due to severe head injuries received.
