Investigation Summary
On October 23, 2017, an employee was using a 22 ton air jack to lift the front axle of a Freightliner tractor with an attached trailer. The employee was under the tractor for an unknown reason. The tractor slipped off the jack and pinned the employee under the tractor. The employee died.
