Investigation Summary
At 2:30 p.m. on December 15, 2017, an employee was walking on the sidewalk after exiting from inside the school. The employee slipped and fell, and fractured his right hip. The employee was taken to the hospital and received medical attention related to the injury. The employee later died due to complications from pre-existing heart disease. The employee slipped and fell due to icy/snowy/slushy conditions on the walking surface causing a fracture to his right hip.
