At 4:15 p.m. on August 4, 2020, an employee was working for a manufacturer of wo od products. He was operating an 1,800-kilogram (2-ton) CM Lodestar overhead hoi st to lift a dock section. The dock section was 13.8 meters long by 1.2 meters w ide (45 ft long by 4 ft wide). Two coworkers were building the dock. They had te mporarily rigged it with a crowbar to lift it up only a few millimeters or inche s. They were eventually going to place a proper sling around it. After placing t he temporary rigging, they went to get carts to place the dock on. The employee went over to the controls and lifted the section a few feet, unaware that the te mporary crowbar rigging was still in place. When he recognized that the temporar y rigging was still in place, he let the dock down. The temporary rigging slacke ned, and the dock section fell. It came down on the employee's legs and ankles.
