Investigation Summary
At 4:15 p.m. on August 4, 2020, an employee was working for a manufacturer of wo od products. He was operating an 1,800-kilogram (2-ton) CM Lodestar overhead hoi st to lift a dock section. The dock section was 13.8 meters long by 1.2 meters w ide (45 ft long by 4 ft wide). Two coworkers were building the dock. They had te mporarily rigged it with a crowbar to lift it up only a few millimeters or inche s. They were eventually going to place a proper sling around it. After placing t he temporary rigging, they went to get carts to place the dock on. The employee went over to the controls and lifted the section a few feet, unaware that the te mporary crowbar rigging was still in place. When he recognized that the temporar y rigging was still in place, he let the dock down. The temporary rigging slacke ned, and the dock section fell. It came down on the employee's legs and ankles. The employee suffered fractures to his legs and ankles. While he was hospitalize d, he suffered a deep vein thrombosis (DVT) clot and died.
