Investigation Summary
At approximately 5:30 a.m. on April 4, 2020, an employee was assessing whether the air-brakes to rear tires to a Manifold unit trailer were locked-up. The employee became struck by the Manifold unit trailer as it was pulled away via a rig by the employee's coworker. The employee received an injury to his head, sustaining a laceration with profuse bleeding. The employee was transported to the local hospital and died at the hospital.
